C.H.A.M.P. Assistance Dogs, Inc.

To be eligible for a service dog from C.H.A.M.P., Inc. you must meet several criteria:

• live in the greater St. Louis, MO, area (approximate 50-mile radius)
• successfully complete the application process
• successfully complete the pre-placement education and training program
• agree to and abide by all C.H.A.M.P., Inc. service dog care and management protocols
• agree to post-placement requirements, including regularly scheduled follow-up visits

IMPORTANT - Completing the application and education program DOES NOT guarantee that you will receive a service dog from C.H.A.M.P. Assistance Dogs, Inc.

If you would like to inquire about obtaining a service dog, please call the C.H.A.M.P., Inc. office at (314) 653-9466. We do not accept applications via e-mail. When you call, we will answer any questions you might have, as well as tell you whether we are open to accept new applications. If we are not open for new applications at the time you call, we will tell you when we expect to reopen, and ask that you call back at that time.
